Refer-a-Friend Program Terms and Conditions

These terms govern the Keyzer Family Dentistry Refer-a-Friend Reward Program (the "Program"), offered in connection with the opening of our Lakewood Ranch location. By participating, referring patients agree to these terms. Keyzer Family Dentistry operates locations at Manatee Ave W, 59th St W, and Lakewood Ranch.
Program period
The Program begins June 22, 2026 and continues until modified or discontinued by Keyzer Family Dentistry at its sole discretion. Rewards are available while supplies last.
Who can refer (the referring patient)
To be eligible to earn a reward, the referring patient must:
- Be a current patient of our 59th St W or Manatee Ave W location who has been seen at that location within the past 24 months, measured as of the date the referral is verified.
- Not be an employee of Keyzer Family Dentistry or the immediate family member of an employee.
Who can be referred (the referred friend)
The referred friend must:
- Be new to Keyzer Family Dentistry, with no prior or existing patient record at any of our three locations.
- Schedule and complete a qualifying first visit at our Lakewood Ranch location. A qualifying visit is a completed full new-patient exam or emergency visit. Consultations, cancelled appointments, and missed appointments do not qualify.
- Not be an employee of Keyzer Family Dentistry or the immediate family member of an employee.
How it works
- The referred friend schedules a first visit at Lakewood Ranch and names the referring patient at check-in, on the new-patient intake form.
- The referred friend completes their qualifying visit. This visit must occur within three (3) months of the date the referring patient is named on the intake form.
- Once the visit is verified, the referring patient is notified and may pick up their reward.
A referral must be recorded at the referred friend's first visit. Referrals cannot be added or credited after that visit has taken place. If a referred friend names more than one referring patient, Keyzer Family Dentistry will determine which referral qualifies, and ordinarily the first patient named is credited.
The reward
The reward is one (1) Philips Sonicare 2300 electric toothbrush per unique referred friend who completes a qualifying visit. The reward is earned by the referring patient only. The referred friend does not receive anything under this Program. Rewards have no cash value and cannot be transferred, sold, exchanged, or substituted, except that Keyzer Family Dentistry may substitute a reward of equal or greater value if the Philips Sonicare 2300 becomes unavailable.
Reward limits
- One reward per unique referred friend.
- A maximum of five (5) rewards per referring patient per calendar year.
Redeeming your reward
Rewards are picked up in person at the referring patient's home office, either 59th St W or Manatee Ave W. Rewards are not shipped or mailed. The referring patient must claim their reward within sixty (60) days of being notified. Any reward not claimed within 60 days is forfeited.
General terms
- Keyzer Family Dentistry verifies all referrals and reserves the right to determine eligibility in its sole discretion. All eligibility determinations are final.
- Keyzer Family Dentistry reserves the right to modify, suspend, or end the Program, or to change these terms, at any time and without notice.
- The Program is void where prohibited or restricted by law.
- Any separate promotion referenced alongside this Program, including any new-patient whitening offer, is governed by its own terms and is not part of this Program.
